Well, how to prevent the worst possible sequence? The answer is the Fool-proofing aka. ポカ-ヨケ(Poka-Yoke). For example, the floppy disk is unable to insert if it was face up.

Well, my wife just ran into the critical human error that she was paniced to call 911 while she drove and stalled Toyota prius during rush hour when she pressed engine power button while driving instead of the radio power button when I called her.

So, these two Power button are bad design. Toyota should consider the Fool-proofing design of it.
